Actually, lieutenant general (LTG) is 3-star general, which is higher than major general (MG) or 2-star general. And there was at least one major general in US at that time.
In 1840, Alexander Macomb was the commanding general of the United States Army and he was Major General at that time.
So your statement "因此史密斯的军衔属于最高的了" is still right because LTG is higher rank than Major General.
